FMLA (FAMILY AND MEDICAL LEAVE ACT) DOCUMENTATION
If you're experiencing a mental health condition that impacts your ability to work, you may be eligible for FMLA leave. At The Way Through Therapy Services, PLLC, FMLA documentation is available upon request and issued based on clinical necessity after a minimum of four therapy sessions. Documentation is completed accurately and ethically based on your diagnosis and treatment plan.
A flat fee of $80 applies for all FMLA forms, with a rush fee of $100 for expedited requests (processed in less than 3 business days). Please allow 7–10 business days for standard processing.
